National Consumer Feedback Network

Have a voice in helping the blindness rehabilitation system change for the better!

The Rehabilitation Research and Training Center (RRTC) on Blindness and Low Vision at Mississippi State University invites you to participate in the National Consumer Feedback Network. The purpose of this network is to help us know what persons with visual impairments really need, what really helps you, and your views/opinions regarding the state/federal rehabilitation program. Your input is essential to the present and future direction of our research and training activities .

To participate in the National Consumer Feedback Network, please complete and return the attached form giving us some information about yourself and how to contact you. This information will be entered into our database so that we can contact you in the future and not have to ask you these questions again. We have many projects that require the valuable information and observations you can provide. We will contact you for interviews and ask you for your participation on our surveys. Of course, your name and information will be kept confidential.

With your help, we believe the work we do together will have a positive impact on people with visual impairments. We value your opinions and experiences. If you have any questions or need additional information, please call us at (662) 325-2001 or TDD: (662) 325-8693 or send e-mail to rrtc@ra.msstate.edu.

You may print the National Consumer Feedback Registration Form, fill in the information, and mail it to the address at the bottom of the page or you may fill out an online version of this form.
